Find Employment Screening is committed to helping survivors of human trafficking. To request to block adverse information that should not be furnished because the items were a result of a severe form of trafficking in persons or sex trafficking, please mail your request to Find Employment Screening at one of the addresses below:

Required Information/Documentation

To assist us in processing your request, the following information is required: 

1. Preferred Method of communication for us to contact you a

2. Proof of Identity

  • Government-Issued Identification Document

    • Driver’s license

    • Government-issued identification card

    • Passport

    • Social Security Card

    • Birth Certificate

 

  • If you have a designated representative submitting information on your behalf, then in addition to your proof of identity, your designated representative needs to submit their proof of identity and a valid power of attorney or some other documentation that authorizes the representative to act on your behalf.

3. Trafficking Documentation:

  • Identified Adverse Items of Information

      - Provide the specific item(s) on your report you are seeking to block.

  • Victim determination

       - A Federal, State, or Tribal governmental entity or;

       - A non-governmental organization or members of a human trafficking task force authorized by a

          governmental entity to make such a determination; or

       - Documentation from a court indicating a determination by a court that you were a victim of human 

       - A self-attestation that you identify as a victim of trafficking that is signed or certified by a representative

          of such government entity, court, or or non-government organization or human trafficking task force

          authorized by a governmental entity to make such a determination
